Asymptomatic Hepatomegaly with Elevated Serum Transaminases – Diagnosis?

A 55-year-old male, from rural India, presented to the clinic for a routine pre-employment visit. He was found to have an asymptomatic hepatomegaly (liver span 22 cm) with elevated serum transaminases (ALT 89 IU/L, AST 65 IU/L, alkaline phosphatase 122 IU/L). The patient had previously been a farmer and currently did not have any systemic symptoms. A computed tomography (CT) scan of his abdomen was performed and is shown in the photograph.
